Veterans asking their Department of Veterans Affairs health care provider about GLP-1 medications frequently run into a wall of brand names that seem interchangeable but are not — at least not for coverage purposes. Ozempic and Wegovy contain identical active ingredients. So do Mounjaro and Zepbound. But the VA treats them as categorically different drugs, and which one you can access depends on what is in your medical record.
Understanding the distinction is the single most important thing a veteran can do before walking into that appointment.
The Two Drugs Behind Four Brand Names
Semaglutide is the active ingredient in both Ozempic and Wegovy. Both are made by Novo Nordisk. Ozempic was FDA-approved for Type 2 diabetes management. Wegovy was approved for chronic weight management and, following a 2023 trial, for reducing major cardiovascular events in patients with established heart disease and obesity.
Same molecule. Different doses. Different FDA approvals. Completely different VA coverage pathways.
Tirzepatide works the same way. It is the active ingredient in both Mounjaro and Zepbound, both made by Eli Lilly. Mounjaro is FDA-approved for Type 2 diabetes. Zepbound is approved for obesity and, separately, for moderate to severe obstructive sleep apnea. Same molecule, different brand, different indication, different coverage.

What the VA Actually Covers — By Diagnosis
The VA covers these drugs through its Meds by Mail program and at VA pharmacies, but the diagnosis in your record is the determining factor. Based on the VA’s current formulary guidance as of mid-2026, here is the breakdown:
- Veterans with a Type 2 diabetes diagnosis can access Ozempic and Mounjaro.
- Veterans with an obstructive sleep apnea diagnosis can access Zepbound.
- Veterans with a diagnosis for the prevention of major adverse cardiovascular events (MACE) — meaning established heart disease with obesity — can access Wegovy.
- Veterans who want these drugs for weight loss without one of those specific diagnoses face a significantly harder path. VA classifies obesity medications as non-formulary except for documented medical necessity, and approval rates for weight-loss-only indications are low.
CHAMPVA — the Civilian Health and Medical Program of the Department of Veterans Affairs — covers dependents of 100 percent permanently and totally disabled veterans; it does not cover GLP-1 medications for weight loss at all. CHAMPVA covers Ozempic and Mounjaro for Type 2 diabetes, Zepbound for sleep apnea, and Wegovy only for MACE prevention — the same diagnosis-driven structure as the VA but with no weight-loss pathway under any circumstances.
What Tricare Covers for Active-Duty Families
Tricare is more permissive than the VA for weight-loss indications. Tricare Prime, Select and premium-based plans cover Wegovy, Saxenda and Zepbound for weight management with prior authorization, provided the member meets body-mass-index criteria — 30 or above, or 27 or above with a weight-related comorbidity; has attempted lifestyle modification for at least six months; and has tried an oral weight-loss medication without success.
Tricare For Life and direct-care-only beneficiaries are excluded from weight-loss GLP-1 coverage as of Aug. 31, 2025. Meanwhile, however, Medicare introduced its Medicare GLP-1 Bridge program for $50 a month.
What Veterans Should Do Before Their Appointment
The most productive thing a veteran can do before discussing GLP-1 medications with their VA provider is to review their active diagnoses in their VA health record through MyHealtheVet and note whether Type 2 diabetes, cardiovascular disease or obstructive sleep apnea appears. If any of those diagnoses are present, the coverage pathway is clearer, and the conversation with the provider becomes simpler.

Veterans who want these medications for weight management without a qualifying diagnosis should ask their provider about enrollment in the VA’s MOVE! weight management program. Documented participation in a structured lifestyle intervention strengthens the case for pharmacological treatment and is often a prerequisite in the VA’s prior authorization process for weight-loss indications.
